Student Records Requests

To request student records, please contact the student's current/last school of attendance.  If the school is closed for the summer, you may request student records by submitting an Authorization to Release Information Form to the Student Discipline and Attendance Office.  A legal I.D. will need to be provided with the Information Request.

If you would like to review your student's records, you may complete form PP-27 Request to Review Student Records and submit it to the student's current/last school of attendance.  A legal I.D. will need to be provided with the Information Request.
