Final Transcripts

If you need a final transcript sent to the college, school or program you will be attending in the fall, you must submit a written Transcript Request Form. A form may be obtained from the Registrar's Office.
  1. Complete the Transcript Request Form. If you are requesting the transcript for a college, indicate how you submitted your application on the form (Common Application, electronically but not through Common Application or on paper). The form must be completed legibly and signed by a parent. Special note: In the "Special Action or Handling Requested" section, please write "Hold for final grades". This will ensure all of your grades will be posted on the transcript.
Additionally, if you would like to request an official transcript for your personal records, follow the same process but make sure to list your address on the form.
