Athletes need a “Ticket to Play” to participate in sports. Print and fill out all five pages of the Athletic Screening History & Physical Exam {Athletic Packet}. The Athletic Packet must be updated each school year. Physicals are good for ONE year from date of examination and cannot expire during the sports season. Only Westview/PUSD Athletic Packet forms are accepted. Bring ALL signed forms to the Athletic Office. ID will be required to receive your Ticket.

  • Complete the Athletic Packet
    • If you carry or use any medication while playing your sport, you must complete the Medical Authorization Form
    • If you don't attend Westview's Physical Night, see your child's pediatrician with pages three and four. Another option, click Places to Get a Physical for a list of physicians near Westview. Make sure all line items {dates, signatures} are completed before you leave the doctor's office; otherwise omissions may cause delays.

  • Turn in your "Ticket-to-Play" Athletic Packet based on the Ticket-to-Play-Timeline for your sport/season. {Sooner rather than later...don't wait until the last day when it's due.}
Step 4
Once you have been selected for a team the following forms are available {depending on need; not all students need to or will use these forms.}:
  • Athletic Exemption from PE is for 10th-12th grade students who are eligible to waive ENS {physical education}.
    1. Parent and Student - Fill out and sign section 1 and 3.
    2. Student - Obtain your Coach's signature once you have been named to the roster.
    3. Student - Turn form into the Athletic Office prior to the deadline and finish the season.
    4. The Athletic Office files the form with the Counselor and Registrar if item #3 is completed.
  • Alternate Transportation Request Form - needs to be filled out at least two days BEFORE an athlete is allowed to take alternative transportation {separate from the team} to or from a sporting event.
    • Parent/Student - Fill out and sign.
    • Student - Obtain your Coach's signature.
    • Student - Obtain Athletic Director's signature.
    • Student - Give the form to your Coach.
  • Private Vehicle Transportation for Students - Requires approval BEFORE a parent is allowed to transport athletes other than their own to or from a sporting event.
    • Parent - Fill out this form and sign it. Bring or email the form, insurance declarations page, and valid California drivers' license to the Athletic Assistant for approval; preferrably two days prior to the event.
