Q: When is Open Enrollment?
A: Open Enrollment is from October 30th, 2017 – November 15th, 2017. All adds/changes MUST be submitted to Insurance Benefits no later than 5:30 pm on November 15th. All completed forms should be sent to the District Office, Attention: Benefits.


 

Q: Where can I find the form(s) to make changes/adds?
A: Forms will not be included in your packet. All forms can be found on the Benefits homepage listed under 2018 Information. Additionally, each site secretary will receive a limited supply. 2018 Active Benefits Enrollment Form


 Q: How do I continue or enroll in the Opt Out Program?
 A: Complete the 2018 Active Benefits Enrollment Form, attach proof of other coverage and submit.


Q: I Opt Out every year, do I really need to complete this form again?
A: Yes, Opt Out information and proof of other coverage is required on an annual basis. Although our fiscal and school year runs from July/August – June, the benefits plan year runs January – December. Even if you just completed this paperwork in September you will need to submit this information again.


Q: How do I enroll in a Flexible Medical Reimbursement Account or Dependent Care Account?
 A: Complete the 2018 Poway Unified School District Flexible Benefit Plan Enrollment Form.


Q: If I’m not changing anything do I have to complete any paperwork?
A: If you do not wish to add/delete self or dependents, change plans, enroll in Hyatt MetLaw, Increase Life Insurance, Enroll/Re-enroll in an FSA Medical or Dependent Care Account and do not wish to Opt Out or Waive coverage for plan year 2018; then NO you do not need to do anything.


Q: How do I add or delete dependents?
A: In order to add or delete a dependent you will need to complete a 2018 Active Enrollment Form. This is an enrollment form not a change form, therefore whatever is reported will replace your current election.


Q: Can I add my parents, nieces, nephews or other family members living with me?
A: No, dependents are defined as: legal spouse/unregistered domestic partner and/or children.
Children are considered eligible if they are:

  • You or your spouse’s / unregistered domestic partner’s biological children, stepchildren, adopted child or foster child up to age 26. Dependent child need not be a student, unmarried, tax dependent or living with parent.
  • You or your spouse’s / unregistered domestic partner’s children of any age if they are incapable of self-support due to a physical or mental disability

 Q: How do I increase my Standard Supplemental Life Insurance Policy?
 A: To increase your Supplemental Life Insurance Policy you must complete the Standard Enrollment/Change Form. These forms are NOT included in your packet and can be found on the Benefits homepage.
· Remember coverage maximums apply. Coverage maximum for self is 5 times your annual salary. Coverage maximum for spouse is 50% of employee coverage The Standard Life Insurance Enrollment Form


 Q: How do I enroll in Hyatt Met Law?
 A: Complete the 2018 Active Benefits Enrollment Form Selecting Hyatt Met Law. You need only complete the top box on page 1, list O.E. – ADD HYATT ONLY under reason for change, mark Hyatt Box under Plan Options and sign the bottom of page 4.


 Q: How do I cancel a Supplemental Life Insurance Policy or Hyatt Met Law?
 A: In order to cancel a Supplemental Life Insurance Policy or Hyatt Met Law you will need to complete OPEN
